Yes, All the Cambridge International AS Level English Syallbuses are accepted in lieu of Class 12 English subject of Indian boards provided students enter for the same in Year 12 of their schooling.

Private medical colleges will not give any fee concession depending on the marks that you earned in 12th. They will allow the seat depending on your entrance exam and they might give any fee concession depending on the rank that you perceived in that entrance exam. So what I would like to suggest is that it is not compulsory to get 90/100 in your 12 state exams, but it would be great if you maintain a decent score.
