It's good to think on this field since fees in Russia are far lower than in India at least for medical.
I think you can seek education loan from bank.

Once you take admission in 1st year of engineering after real lengthy process, it's next to impossible to shift college in second year. What is reasonably possible is to change branch in same college if seats are available.

It's really nice to see your inclination in this rare subject. This Institute is part of TIFR and eligibility is graduation in any subject with 50% marks but candidate should have interest in this subject.
