Step 2: Fill out the form online and take a printout of it or download the form and fill it offline.
Step 3: Send the application form to the university campus with a DD of INR 1,200 drawn in the favour of SRMIST, payable at Chennai.
Step 4: Candidates must write their name and address on the reverse of the DD.

Candidates seeking admission to the MBA at SRM School of Public Health are required to have following documents during the time of admission:
- Class 10 and Class 12 mark sheets and certificates
- Degree and certificate of Bachelor's and Master’s degree (as applicable)
