You don't have to worry about filling out forms for college now. It starts around April-May. So, when you are in class 12th, then just make sure you have a clear concept of the syllabus you learned in +2. So, you can do well in entrances apart from the Merit based system. Look out for entrances, as they take place during May and as far as DU is concerned, the cut offs come out in late May or early June.

Fill in your real surname, the one that is mentioned on Aadhar card, to avoid confusion, as your Aadhar card is the most recent document you have, and will be the most likely to be asked for, when asked to produce proof of identity. Regards.
