I would necessarily say St Xavier's College Mumbai is much better than say DU. As here you get an all-round exposure apart from studies. And I personally believe that the kind of personality development you would get here at Xavier's is beyond any colleges of DU unless you go to Stephen's, LSR, Hindu etc.

No requirements. You only need to clear the cut-off, which ranges from 96-99% for Non-Christian General category. You can be from any subject background.

Yes,
You can apply to bachelor of Pharmacy form any institute. So, keep your Diploma of Pharmacy ready to get admission. Even you can apply if you have a provisional certificate of the same. Note, you have completed your diploma of Pharmacy in all aspects like 500 Hrs hospital training and all.
