What percentile is required in CAT for getting a call from IIM- Sirmaur, Amritsar & Kashipur in individually?

First of all, getting an IIM call depends on a lot of factors other than CAT percentile like number of applicants, seats available etc. Officially IIM Amritsar requires 90 percentile and IIM Sirmaur and Kashipur both requires 95 percentile for a general category student to get a call for GD PI. But

Yes, you are eligible but you should have maths in 12th.

Yes, you are eligible. But, you have to clear the university exam which is to be done in college itself.

Evening/distance/online/part time engineering courses are not valid any more as per the latest supreme court verdict, hence, I would recommend you to join regular B.Tech course for better career prospects.
