Excluding maths in the best of four list, your percentage is 84%. For general category, there is hardly any chance of getting honours. You may get B.Sc. (Physical Sciences), if lucky.

To be frank, getting courses like Physics and Chemistry honours, there is a slight chance. However, for all the biology based courses and courses like Physical Sciences, you can be assured about getting admission.

There is as such no option in applying for specific colleges in merit based courses. However, when the cut-off list is out, select the course only at north campus colleges.

Well, since you did not mention about the marking scheme, so, let's assume two conditions-
1. CGPA system - Mostly, the board mark sheets have a formula for converting CGPA to percentage. If not, then multiply by 9.5.
2. Practical and theory ratio not 30:70 - Fill the detail as it is. There is some changes in these cases which I am not sure of.

In the form, all five subjects are to be filled. The best of four shall be taken when you take admission in a college.

There is either any error in detail. If this is not the case, then there is probably an overload on the site. Try again and they will be submitted.

Well, you can apply for DU. The fee is only 100 Rs and your percentage is worth a try in Delhi University. Though I am quite sure that getting any popular course is a rare chance.
