No, candidates do not need to fill separate application form. The XAT application form has an optional field to select the preferred XLRI courses. Candidates have to pay an additional fee for the same.

No. XAT application fee cannot be refunded. If a candidate makes a mistake during registration, or if they fail to appear for the exam, registration fee is not refunded.
