Should you decide to quit OUAT Bhubaneswar after 1 year to join one of the government medical colleges, then you would most probably face a discontinuation bond penalty. In MBBS courses in Odisha this penalty is typically a fine amounting to half a Lacs of rupees. Moreover, you may not have back some fees paid, with exception of caution money. These are specifically provided under the bond that one signs during the time of admission and is aimed to persuade the student against leaving the seat vacant midway through the course.

As you have 78% in 10th, 74% in 12th, and 76% in the OUAT scorecard, your rank would likely fall within the mid-range depending on the overall competition level for the specific course you are applying to. With a 76%, you have good chances of getting admission at OUAT, as your rank is decent to take admission in various courses. Although it depends on the course also, which you are applying for, because different courses have different seat intakes and competition, and the cutoff varies accordingly. Your rank is meeting the expected cutoff ranks for the next sessions, so you'll generally not face much trouble in getting the admission.
