Hi Rahul,
For admission to First Year B. Optometry course the candidate should successfully completed H.S.C./10+2 or equivalent with min 45% marks in Physics, Chemistry, Biology or Mathematics & Pass in English.
Every institute has it's own course specific fee structure. In order to know the exact fee it would be always advisable to browse through the official website of the institute where you wish to apply.
