If it's the question about the low is, then I suggest you to introduce him to some practical work, like getting him into an ITI course. There he will be able to gain practical knowledge with proper instructions. If he is refuses to do that, then Try whatever he is good in like painting, singing, dancing, music, sports, etc. And support him to do the same. Career wise it is good but he must be really good at those things. You can join a Diploma course in physiotherapy. I would also suggest you to do a Bachelors, if anyways you want to pursue physiotherapy as long term career. It is indeed a field with lot of scope and opportunities.

Best thing to do would be to discuss with your brother and help him figure out options. After class 10th, 3 broad categories to consider are, Arts, Commerce, Sciences. Arts - Psychology, journalism, media and entertainment, writing to name a few. Commerce - Accounting, Auditing. Sciences- traditional sciences like B.Sc. Engineering and medicals. Let him prepare for entrance exams too if it is engineering or medicals. In any case, check his interest, skill set and ability and then make a decision.
