PhD programme: You can pursue once you complete your Master degree not after Bachelor degree.
You cannot pursue 2 regular degree programme from same university.
If your diploma is equivalent to Master degree, then only you can apply for PhD according to university's eligibility and admission process.

You need to check every university's eligibility and admission process for this programme.
Some universities may offer you with below 50 percentage . Do check the Shiksha website for top colleges related to same.

Yes, you can pursue 2 different programme from 2 different university in regular and distance mode.
LLB is a fulltime course so accordingly you can pursue MA as distance mode with IGNOU.
