You must take the SAT and TOEFL for Bachelor degree studies in USA. Report your test scores to universities you want to apply and check their websites for admission requirements. You must also submit a personal statement, letters of recommendation, etc. The application process is online, so check their websites.

For 2017 batch, it is too late. Admissions are almost closed, but you might still find a few options. Visit university websites and check if deadlines are still on. You need to start the process a year ahead. You must take SAT, TOEFL and also visa process takes time after you get the admission offer.

Yes, you can apply but after taking the GRE. You must have a very good score as your Bachelor degree grades are not too high.
