There is no course which can guarantee you a PR. If you need a PR, you have to get a job first. Now you decide what kind of job you want to do and based on your own career plans and interest you have to decide what to study. This is not something others will tell you. Know your own interest and plan accordingly.

Your grades are way below average to study in Canada. A minimum 75% is required. Also, there is no MBBS as such, you have to apply for a Bachelor's degree, clear MCAT and start your medical studies.

Keeping in mind your grades and backlogs, try for countries like Australia, UK, Ireland, Poland, Italy, Spain, etc. Getting into a top ranked university may be difficult but if your SOP, LOR and overall profile is good with outstanding projects and internships you may have a chance.
