For Bachelor's degree an IELTS of 6 with no section less than 5.5 is required. For Masters level an overall score of 6.5 with no section less than 6 is required.

The best country to do Masters depends on a lot of factors like: financial ability, courses that you want to pursue, your relatives living abroad, your plan post graduation, etc. Canadian Universities are considered one of the best for most courses whereas Germany is best for engineering studies.

It's just like betterment exams, if you retake and score better than the previous attempt then the current score which is better will be considered. And, if you score less than the previous attempt then your previous score will be considered.

Try in Australia since your academics is low for Canadian colleges. Also your IELTS score, you should retake IELTS. For maximum colleges, abroad, they require an IELTS score of 6.5 with no section less than 6. Email the colleges and check if the present score can be accepted by them or if any bridge course is available.

For PG level course, universities require an overall score of 6.5 with no section less than 6. You must also meet the academic requirements. Every university has different requirements, so check the websites before applying.

Universities abroad have bridging courses or foundation programs in English, designed for students not meeting English language requirements. You must visit college websites and check for these details on the same. Please email them for IELTS requirement and such bridging courses, colleges will guide you accordingly based on your profile.

The admission process is different for both Bachelors and Masters courses. You did not mention what do you want to study.
Yes, IELTS or TOEFL is required. If you are applying for undergraduate studies, German language is also required.
