Hello Sunil! Only having a good CAT score(99+ percentile for older IIMs) isn't enough. You also need to meet their eligibility criteria, have good academic scores, and perform really well in their What-PI rounds to get selected.

It does not matter how much you have scored in graduation. I would recommend you to prepare and give CAT exam. If not 90 percentile, even if you score 70 percentile also, there are many good MBA colleges you can take admission. If possible, you can also give other MBA exams like MAT, CMAT, XAT, SNAP, IIFT, NMAT, etc.

Yes, you can apply. ISME requires any entrance exam score. If you have not given any still just give it a try.

Yes, you can definitely for PGDM course as the requirement of scores for the various colleges is distributed along as 50% + grades in 10th, 12th and graduation and a good MAT score. So, I would suggest you to prepare well for the MAT exam and definitely look forward for your preferred opportunities.
