You need to get in touch with the university or college of your choice. Ask them about the eligibility criteria for scholarships are they are different for each institute.

Sweden education is great but very much expensive and don't choose Poland as per job prospect. Poland is very poor. You can choose Italy, Spain and Finland, but increase your percentage in B.Com. Germans need excellent academic record.

Lateral entry option is only possible in India. At the best, to study abroad, you can shortlist the countries first and then universities. Write to them about the courses you hv taken in your diploma and request them to see if the credits can be given for those or not.
For admission abroad you will also need SAT and TOEFL/IELTS 6 band.

Admission requirements vary with the level of study and also depends on the university. Bachelor degree applicants are required to have at least 5.5 in each section while master degree students must have a score of 6.5 overall with no band less than 6.
Please note that this is just a minimum score requirement. Universities however, have special foundation programs for applicants not meeting English language requirements. For this, please contact the individual universities.
