To study in Poland, you particularly don't require IELTS or TOEFL. Some of the universities may ask for it. As it's not an English speaking country but from visa interview point of view, it's good to appear in order to support your file.

Are you applying for a Bachelor's or Master's degree? As with 6.5 IELTS, overall with no section less than 6.0, you have probable chances of getting admission in Canadian universities.

It particularly depends on the country you're looking forward to. As many European countries don't ask for IELTS as a mandate whereas for UK, Canada and Australia, it's a mandate to have one of the preferred English proficiency test.

Please specify the country you intend to study and the desired course.

Write a good convincing statement of purpose and also collect letter of recommendation from teachers/professors, who have taught you relevant subjects of your further university application.

A question which every admissions officer will ask is, why the 3-year gap? Especially, if it is a University which is situated outside India. If you do not have a logical explanation then you may not get admission. You can apply and try if you get through then good, if not there are enough good Universities and Colleges in India where you can do your Masters.

IELTS is mostly a requirement from the school. Under certain cases you can apply to a school without IELTS. If you have completed your education in an English medium school/college, or if you have spent good number of years or even studied previously in an English speaking nation chances are that the university may waive off the IELTS requirement. You need to write to the concerned university directly. Once you receive an admission offer, you will apply for the visa. Hope this helps. Feel free to revert.

GRE and IELTS requirements varies, so I will request you to visit the individual websites for eligibility criteria. It is recommended that you have very good academic grades, GRE score of above 310 and IELTS of 6.5 as these are some of the most competitive universities.
