You can apply for direct PR, but it would be better, if you apply for study visa. You can choose a one year course, then you can have more chances for getting visa, and then you can apply for PR.

Yes, PG Diploma or graduate certificate courses. The duration of most of the MS programmes in Canada is one year. You must be very careful while applying as you already hold an MCA degree, choose your next PG course carefully and keep your career interests on priority. Your SOP and LOR will also be considered for admission. Every university has its own requirement and deadlines, please visit university website for these details.

Through your profile, you should demonstrate sufficient research work/ interests, any kind of work experience in hospitals, etc. You must basically have the relevant background. A year of full time experience in the health sector will help.
