- Mark's memorandum cum pass certificate of Diploma Course (or) Course Completion Certificate of Diploma issued by the Principal of the Polytechnic.
- Proof of local status (last 7 years) through a study or residence certificate.
- Payment acknowledgement (Transaction ID/UTR) paid through online for INR 1400/- for General and INR 700/- in case of SC/ST/PH candidates (or) in the event of late fee paid INR 2,800/- (INR 1400/- in case of SC/ST/PH).
- Certificate issued by the concerned authorities in the case of Special categories like PH, NCC, Sports and CAP.
- One self-attested passport-size photograph.
- Latest Caste Certificate (in case of SC/ST/BCs) issued by the Mandal Revenue Officer (not older than 6 months).
