You need photo, signature, 10th certificate, email, phone number. Make sure your files match the sizes or NTA can reject.
Yes, NTA gives you chance to fix your NCHMCT JEE form before admit card comes out. You can change some mistakes like name, contact, category, DOB. But do it before last date, or it stuck like that.
Mostly, all details can be edited in the online application form correction window of NCHMCT JEE 2026. Candidates can change any details until the online correction period. Once the window closed no details will be changed.
The candidates must fill out the form with utmost care. The candidates are advised to ensure the following points before submitting the form:
- Whether they fulfil the eligibility conditions for the test as prescribed under the heading ‘Conditions of Eligibility’.
- That they have filled their gender and category viz General/OBC (non-creamy layer)/SC/ ST/Gen-EWS/ Male/Female/Third Gender, in the relevant column correctly.

Candidates can follow steps below to correct details in application form:
1. Login using application number and password.
2. Access link for application form correction and click it.
3. Application form opens fields will be available for editing. Edit details.
4. Save details and submit changes.
5. Download revised confirmation page.
You can fix your name, address, phone number, category, PwD status, school marks, date of birth, and even preferred exam centres. Not everything can be changed, only what NTA allows in correction window.
Fill your category, address with PIN, and choose exam cities. Then pay fee online through card, net banking, UPI, wallet, whatever works. Keep fee receipt safe. After payment, print acknowledgment page for future.

Instructions for Photograph:
- Candidates are advised to take 6 to 8 passport size coloured photographs with white background.
- The scanned clear passport photograph must have 80% face (without mask) visible including ears against a white background.
- Polaroid and Computer-generated photos are not acceptable.
- The photograph should not be with caps or goggles. The photograph should cover 80% face (without mask) visible including ears against a white background.
- Spectacles are allowed if being used regularly.
Instructions for signature:
- The candidates are required to upload the full signature in running handwriting in the appropriate box given in the Online Application Form.
- Writing full name in the Box in Capital letters will not be accepted as signature and the Application Form will be rejected.
- Further, unsigned Online Application Forms will also be rejected.
- The candidate should put his full signature on white paper with a Blue/Black Ink pen and scan it for uploading.
