- Passed 10+2 or equivalent from a recognized board with Physics, Chemistry, and Mathematics is eligible for Bachelor of Optometry, and Bachelor in Audiology and Speech Language Pathology.
- Passed 10+2 or equivalent from a recognized board with Physics, Chemistry, Computer Science / Statistics/ Electronics/ Psychology is eligible for Bachelor in Audiology and Speech Language Pathology only.
- Obtained 45% marks (40% for reserved category).

Admission to the Paramedical courses in UP is based on entrance test. Common Paramedical Entrance Test (CPET) Uttar Pradesh is conducted for admission to all government / private paramedical colleges affiliated to ABVMU and SGPGIMS Lucknow, KGMU Lucknow, and UPUMS Saifai. UP CPET is state level exam. Candidates need to appear for the entrance test, qualify the test and participate in the counselling process. Based on the marks obtained in the entrance exam merit list are prepared. Students having name on the merit list have to participate in the counselling process.
