- Valid Government ID proof: Aadhaar Number (last 4 digits)/Election Card (EPIC No.)/Passport number/Ration Card Number/ Bank Account Number/PAN/ Other valid Govt IDs
- Date of Birth (as mentioned in Class X Board Certificate)
- Educational/Qualification details
- Scanned clear passport photograph in JPG/JPEG format (size between 10 kb–200 kb)(without mask)
- Scanned clear signature in JPG/JPEG format (size between 4 kb–30 kb)
- List of City of your Choice (Refer to DAVV CET 2022 exam centres)
- Bank account details for payment of a fee
- A valid e-mail Id
- Active mobile number
Steps to fill DAVV CET application form 2021
Step 1- Visit the official website- davv.nta.ac.in
Step 2- Click on the tab ‘DAVV (PG) CET- 2021 New Registration’
Step 3- Screen will display the application form
Step 4- Start filling the application form by entering the following details:
- Personal details
- Qualification details
- Image upload
Step 5- Pay the application fee of Rs. 1500
