I have poor Bachelors, with less than 50% marks. Which top MBA/PGDM colleges can I realistically aim? I'm aiming 95+% in XAT.

Bachelor's results do have an impact on your MBA selection but it doesn't mean you can't get a good MBA college. You must try to study hard so that you can get a good percentile in XAT these are a few colleges if you score 90% or above.
